Doctors & Dentists are comming into the 21st century by putting their new patient forms online to be downloaded or filled out right online. No more waiting in the doctor’s office or having to come in early just to fill out forms. These can easily be printed and brought into the doctor’s office on the first visit or conveniently faxed or emailed as the patient wishes. Very nice idea.
